 How do people do this, what with the dangerous cut aspects and everything?

When I've trained Thai it was just on pads and then no knees or elbows to the head in sparring.

In MMA, I guess you could use elbow pads for GnP?

Ideas welcome!

 regarding knees in sparring I let my guys throw knees to the body and legs but not to the head. if you want to knee the head we do so showing "intent" but pulling  the technique to let the guy know he was in danger.

elbows - without elbow pads on, we prohibit them in sparring, but when you are doing clinch work you can thro light elbows to the neck,collar bone area. I learned this from a friend who travels to Thailand frequently and trains.  with the elbow pads (like the Windy ones) you can actually elbow with relative safety. assuming you're not fighting like two animals.
